How to Apply

If you are currently enrolled in an Ontario secondary school, you must complete the application process together with a guidance counsellor or an administrator at your current school.

Step 1: Speak with a guidance counsellor or administrator at your school about transferring to Monsignor Fraser College.

Step 2: Have your guidance counsellor forward your credit counselling summary and completed School Referral Form to the campus location you wish to attend (You can find links to all forms below in step 3).

Step 3: Download and complete the forms below.

Step 4: To complete the application process, you must call the campus you are interested in attending to make an appointment.

Step 5: Make sure you bring the items listed in the checklist below to your appointment.

  • Credit Counselling Summary (forwarded by guidance counsellor)

  • Transcript (if you attended a private secondary school in Ontario)

  • Index Card (forwarded by guidance counsellor)

  • Individual Education Plan (forwarded by guidance counsellor if applicable)

  • Completed Registration Form (signed by parent/guardian if student is under 18 years)

  • Proof of Address (must be one of the following: utility bill, bank statement, phone bill, or lease)

  • Photo ID (can include a driver’s licence, health card, permanent resident card, etc.)

  • $25.00 Activity Fee (cash only and non-refundable)

If you have attended an Ontario secondary school previously but are not currently enrolled, you must request a copies of the documents listed below from your former school in order to apply to Monsignor Fraser College.

Step 1: Contact your former secondary school and request a copy of your credit counselling summary and transcript.

Step 2: Download and complete the Registration Form.

Step 3: To complete the application process, you must call the campus you are interested in attending to make an appointment.
